How to Request the Communication Assessment Service
The process is simple. Print out our Assessment Form for your client to fill in and fax it to us with their audiological report, type of hearing aids and your case notes. We will prepare a comprehensive 5 page report for case management and documentation.
We perform hundreds of communication assessments every year and prepare these reports that identify communication issues and potential solutions using technology and communication strategies. There is a fee for the reporting service. Please call for current rate.
