have obtained health information about you to provide our specialized services,

We protect the privacy of your personal and health

How We Use or Share Information
Orders: When you place an order with us,
we request your name, mailing address, shipping address, email address,
telephone number, credit card number and expiration date. We use this
information to process your order as quickly and efficiently as possible. We

Newsletter Subscription: When you request a subscription to our newsletter, we request your
name, company information, mailing address, and email address. You will receive
email notification when the newsletter is available online. Your information is
not used in any other promotional activities unless you have indicated such to
us in separate correspondence. Each email provides the opportunity to be removed
from the newsletter email list.
Communication Assessment
Reports:
This specialized service is requested
by vocational rehabilitation counselors, disability service providers or human
resource managers and you supply the information needed to perform an
assessment.
You are providing health information
when you describe your hearing loss and share your audiogram. We protect your
personal health information and only share the information you provide as
described below.
If your vocational rehabilitation
counselor, disability service provider or human resource manager requests a
communication assessment, you will be asked to fill in a questionnaire called
the "Communication Assessment Form". We also obtain a copy of your audiogram to
perform this assessment.
The information from the questionnaire
and audiogram is used by an assessor of Beyond Hearing Aids, Inc.
to prepare a report describing and summarizing the details you have given in the
report pertaining to your communication demands, especially on the job.
During the course of service, we may
need to call or share information from:
- you to obtain additional details or
- your audiologist to obtain technical information related
to the specific hearing aids or
- your counselor, service provider or human resource
manager to obtain information related to your job functions or technical needs
on the job.
The final assessment report is mailed
to the professional who requested the service. We cannot release this report to
anyone but the professional who requested it. You must request a copy from the
professional you are working with. We do not use any of the information we have
obtained from you for any other purposes than to produce the report.
ALD Services from your Hearing Healthcare
Provider As specialists of assistive
listening and alerting devices (ALDs), we work with many hearing healthcare
professionals to provide services directly to their patients. If you have
requested assistance with ALDs, you can be assured that your privacy is
protected.
